Thank you for your interest in participating in the "Nurse Practitioners’ Responses to the Opioid Epidemic" study. Please read the study description below and answer the questions that follow it.

Study Description: The U.S. is in the midst of an opioid epidemic. The number of opioid overdose deaths doubled from 2010 to 2016 and now kills more people per year than car accidents or breast cancer. Nurse practitioners are an increasingly important part of the healthcare workforce, but little is known about how they have been responding to the opioid epidemic. This study asks about your prescribing of opioid medications as well as medications to treat opioid dependence or overdose. We also seek to learn about how nurse practitioner's relationships with physician colleagues and the work environment influence their practice.

Your participation: The survey should take about 15 minutes to complete. Your participation is voluntary and you can stop at any time. We have designed the survey to keep responses anonymous and confidential. Once you have completed the survey, you will have the opportunity to be entered into a drawing for one of five $50 Amazon gift certificates. If you wish to be entered into the drawing, you will be linked to a separate survey to provide your email address. Your survey responses and email address will be stored separately and cannot be linked. Please complete the survey only one time and in one sitting (it cannot save your place).

Disclosures and contact information: This study represents the Doctorate of Nursing Practice (DNP) Project of Andrea Caputo in collaboration with faculty at the MGH Institute of Health Professions, including the principal investigator, Dr. Alex Hoyt. None of the investigators have financial ties topharmaceutical or healthcare companies that would stand to benefit from the results and this study has not received outside funding (money for the gift certificates comes from the investigator's own funds).
