Thank you for taking the time to complete the following confidential survey. The survey will ask you questions related to your HIV prevention care practices, as well as your awareness, attitudes, and perceived barriers of pre-exposure prophylaxis (PrEP) implementation.

Agreement to Participate in a Research Study

We would like to ask you to take part in a research study called "Evaluation of pre-exposure prophylaxis implementation for HIV prevention in primary care settings" that will explore primary care providers' engagement in sexual health discussions with patients, as well as pre-exposure prophylaxis (PrEP) attitudes, awareness, experience, and perceived barriers to provision through the completion of this survey and/or interview.

You may be asked to complete a one-time survey that asks about your demographic information, engagement in sexual health discussions with patients, and PrEP attitudes, awareness, experience, and perceived barriers to implementation. This survey will probably take 10-15 minutes to complete. There are 52 questions we would like you to answer by choosing one answer for each question by marking an "X" next to the most appropriate option or filling in your response. At the completion of the survey, you may be asked if you are interested in providing your contact information to participate in a qualitative interview at a later date. If you are interested, you will be directed to a new page to enter in your contact information. This information will not be connected to your survey responses.

If you participate in the qualitative interview, we will ask you questions about your engagement in sexual health discussions with patients and barriers/facilitators to PrEP implementation. Your participation in this interview will probably take around 30-45 minutes. You will be asked about 1) demographic information, 2) engagement in sexual health discussions with patients, 3) HIV/STD screening practices, 3) PrEP awareness and experience, 4) perceived barriers to PrEP implementation, and 5) perceived facilitators of PrEP implementation.

There are no questions that should cause you any discomfort. Your taking part in this research study is completely voluntary. You may decline to participate in the survey and/or interview.

Your participation in this study may not benefit you personally. We are hoping these completed surveys/interviews will provide information to further facilitate the implementation of PrEP in primary care settings in order to increase access to preventive HIV care in Rhode Island.

The questionnaire from this study will be kept confidential. None of the information you provide will have your name or any number on it that will identify you personally. Interviews from this study will also be kept confidential. The interviews will be tape recorded and transcribed into paper copies. Your name will not be kept with the information you provide in the interview.
