Note: 1   You  ---MUST--- be a DA "Certified" WebEx host to use this form and/or schedule a meeting
Note: 2   There can be only one "Host" for each meeting

Are you a DA "Certified" WebEx host? *
Have you attended one of the DA Host Training Sessions?  If not, please attend a DA WebEx Host Training before scheduling a meeting.    Please note:  If your name is on the DA WebEx Certified Host list the request will not be processed.
Is this request 5 or more working days before the mtg? *
You need a allow for "at least" 5 working days for the volunteer staff to process your request
Please ensure you have left 1/2 hour before and after your meeting *

DA Affiliation EMEA/AP or Americas *
Date and Time in Wash. DC time *
(All meetings are scheduled in USA Eastern Time Zone: either EST or EDT depending on the date. Also, the output is in 24-hour time.  As such:  12:00AM is 0000, 12:00PM is 1200)  The information entered here is the ONLY data used to schedule your meeting.  ***NOTE:  The following field "Meeting time in other local Time Zones" is for information purposes only and should be validated by the Requester/Host.***

Meeting Host *
This person "must" have taken the DA WebEx Host certification course.  Their name/email address must appear on the DA WebEx Host list.  If not, the request will not be processed.

(Email of the Meeting host as noted above  (e.g. brad@ nowhere.com)
