This link is LIVE to all incoming Middle School students for the 2019-2020 school year as of 12 noon on May 15, 2019.  Please carefully complete all of the information and once you hit SUBMIT,  you will be emailed a receipt of all of the information that you submitted for your records.  Please consult the Athletic/Activity handout that you should have received approximately 2 weeks ago to look at all of the options available. A reminder that some of the options WILL fill up quickly (Spring Tennis in particular) and the rosters are done strictly on a first-come-first-serve basis. Students on a roster from the previous year are not guaranteed a spot the following year.  If you do not submit this form promptly, your child may be placed on a wait-list in the event that the maximum spots are filled.  Your quick response is greatly appreciated.

Please remember that when making your choices,  there is a 1-season athletic requirement each year. Students who might choose studio art or drama (or similar non-movement options) in the fall AND winter MUST make an athletic choice in the spring. Students who apply for an Exemption must still comply with SSA's 1-season requirement and must complete the necessary information on a separate handout.

You will receive a confirmation email once the information has been submitted (PLEASE make sure that you check the email for accuracy).  PLEASE NOTE:  the confirmation email will NOT tell you if your child will be placed on a wait-list for an activity that has been filled.  You will be notified once school begins if this is the case.
