Title of Project: The impact of COVID-19 on therapy services, function and participation for children with disabilities.
Request to Participate in Research
We would like to invite you to take part in a research project. The purpose of this research is to understand how the COVID-19 pandemic has affected delivery of therapy services, technology use, and participation for children with disabilities.
You must be at least 18 years old to be in this research project.
The study will be administered through REDCap, a secure online database. If you decide to take part in this study, we will ask you to fill out a survey about how your child’s therapy services, use of technology, and participation in daily activities may have changed during the COVID-19 crisis. The survey will take about 15-30 minutes to complete, depending on the number of different therapies your child receives and devices they use.
There are no foreseeable risks or discomforts to you for taking part in this study. There are no risks
associated with participation in this study, and it is very unlikely that taking part in this survey will cause you any discomfort. You may not be able to answer all the questions, and you do not need to answer any questions that you do not wish to answer. If you become uncomfortable at any time, you can stop the survey.
There are no direct benefits to you for participating in the study. However, your answers may help us to learn about ways to maximize the effectiveness of remote therapy and use of technology for children with disabilities. Information from this survey will also help us understand how to best support families of children with disabilities during these unprecedented times.
You will have the option to enroll in a raffle for one of five $50 Amazon gift cards after completing this survey.
The information you provide in this survey will be kept confidential. Data will be stored in a secure, online database. Your participation in this study is anonymous. If you choose to provide your contact information at the end of the survey, this contact information will be stored separately from your survey responses and will not be associated with your answers to survey questions. Any reports or publications based on this research will use only group data and will not identify you or your child as being affiliated with this project.
Your de-identified information could be used for future research without additional informed consent.
It is possible that respondents could be identified by the IP address or other electronic record associated with the response. Neither the researcher nor anyone involved with this survey will be capturing those data. The decision to participate in this research project is up to you. You do not have to participate and you can refuse to answer any question. Even if you begin the study, you may withdraw at any time.
